This could be your opportunity to work collaboratively as part of an interdisciplinary team that shares the goal of delivering care to the community through early detection and intervention to reduce unnecessary hospitalizations.

Care Co-Ordinator — registered Social Worker or Allied Health professional to assess health and well-being needs and goals. Offer effective care coordination during transitions of care. Form and retain strong relationships across the health, well-being and social services sectors or;
Physiotherapist– Have a Bachelor of Physiotherapy (or equivalent qualification), with current APC (Annual Practising Certificate), no restrictions and registration with the Physiotherapy Board of NZ.
